Forms

The following forms will assist you in placing your Questcor order.
Completed forms may be faxed to Questcor at 708-458-7717.

• Quote Request Form – To request a no-obligation quote, please complete this form and fax to Questcor. An online version of this form is also available.
• Customer Information Sheet – If you wish to place an order after receiving your quote, this form must be completed and faxed to Questcor.
• Customer Credit Form – If you wish to place an order after receiving your quote, this form must be completed and faxed to Questcor.
• Tooling Transfer Notice – To transfer existing tooling/dies, please complete this form and send to your current tooling location. This notice will provide instructions on where and how to forward tooling to our location.
• Visit Request Form – To schedule an on-site visit by a customer service representative, please complete this form and fax to Questcor. A representative will then contact you to arrange an appointment.
